The NIST AI Risk Management Framework was published in January 2023. In the three years since, it has become the de facto reference standard for U.S. organizations building AI governance programs — cited in regulatory guidance from the SEC, OCC, HHS, and DoD, and increasingly expected in vendor due diligence questionnaires and audit examinations.
The problem is adoption without implementation. Organizations across regulated industries have "adopted" the NIST AI RMF the way they adopted other compliance frameworks: they mapped their existing activities to it, declared alignment, and moved on. That's not implementation. That's documentation theater — and regulators are increasingly equipped to tell the difference.
What Is the NIST AI RMF, Actually?
The NIST AI Risk Management Framework is not a checklist, a certification standard, or a compliance requirement. It is a voluntary process framework — a structured description of what a mature AI risk management program looks like as a continuous operational cycle.
This distinction matters for regulated industries. The NIST AI RMF does not certify you as compliant with any specific regulation. What it provides is an organizational architecture for AI governance that maps well to existing regulatory expectations around model risk management (SR 11-7), clinical AI oversight (FDA guidance), and supply chain security (CMMC).

The functions are interdependent and sequential — but they're not linear phases you complete once. They operate as a continuous cycle, with Govern serving as the persistent organizational foundation that enables the other three to function.
Why Regulated Industries Need a Different Approach
The NIST AI RMF was designed as a universal framework — applicable to startups and Fortune 500 companies, high-risk medical devices and low-stakes content recommendation systems. That universality is its strength as a consensus document. It is also why regulated industry implementations require translation, not just adoption.
Healthcare, financial services, and defense operate under sector-specific regulatory regimes that impose obligations the general NIST AI RMF guidance does not explicitly address. Implementing the framework in these environments requires mapping AI RMF functions to your existing compliance obligations:
- Healthcare: HIPAA Privacy and Security Rules, FDA Software as a Medical Device (SaMD) guidance, OIG compliance expectations, and state-level AI bias requirements all create constraints on AI deployment that the RMF's Govern and Manage functions must accommodate.

The organizations that implement NIST AI RMF most effectively in regulated sectors don't treat it as a standalone framework. They treat it as the organizing architecture that coordinates their sector-specific compliance activities into a coherent AI governance program.
4-Step Implementation Roadmap
Implementation is not a single project. It is an organizational capability-building process that, done correctly, results in a self-sustaining AI governance function. The roadmap below describes the sequence for organizations starting from scratch or from a framework-mapping-only baseline.
Before mapping or measuring anything, build the organizational structure that makes governance sustainable. This means: chartering an AI governance committee with defined authority and meeting cadence; assigning named individual accountability for each AI system (not team accountability); documenting escalation paths for AI risk decisions; and establishing policies for AI development, procurement, and deployment. Most organizations skip this step or treat it as documentation work. It is not — it is the organizational redesign that makes everything else function. Without named accountability structures, risk assessments produce findings with no owner and monitoring produces alerts with no response.
You cannot govern what you cannot see. Step 2 is a structured discovery exercise: identify every AI system in production, development, and procurement; document each system's use case, decision authority, data inputs, and affected populations; and classify each system by risk tier (low, medium, high, critical) using criteria appropriate for your regulatory context. For regulated industries, risk tiering must account for regulatory exposure — a low-complexity model used in credit decisioning is high-risk by definition under ECOA/fair lending obligations, regardless of its technical complexity. The AI inventory is a living document, not a project deliverable. It requires a named owner and a defined update process.
For each AI system in your inventory — prioritized by risk tier — establish the measurement program that generates continuous evidence of governance. This includes: bias and fairness testing appropriate to the system's use case; performance monitoring with defined thresholds and alert procedures; model validation cadence with documented methodology; and third-party AI vendor assessment processes. The Measure function is where most regulated organizations have the largest gap. They have policies that say "models must be monitored" but no operational monitoring infrastructure. Measure requires tooling, process, and ownership — not just documented intent.
With governance infrastructure, an AI inventory, and a measurement program in place, the Manage function closes the operational loop. This means: documented incident response procedures for AI failures (tested annually); model remediation processes when monitoring flags threshold breaches; a regular risk reporting cadence to the governance committee and executive leadership; and a continuous improvement process that incorporates lessons from incidents, regulatory updates, and framework evolution. Manage is not a one-time configuration — it is the operational heartbeat of the AI governance program.
Common Implementation Pitfalls
After working through NIST AI RMF implementations across healthcare, financial services, and defense organizations, the failure modes are consistent. These are the five most common pitfalls — and why they're worth actively avoiding.
Organizations implement Govern as a project milestone — draft policies, assign a committee, declare complete. Govern is not a phase. It is the continuous organizational infrastructure that all other functions depend on. AI governance committees that meet quarterly but have no authority to pause AI deployments are not functioning governance structures.
Most organizations' first AI inventory captures internally developed models but misses vendor-provided AI in enterprise software, embedded AI in clinical decision support tools, and AI capabilities added via third-party APIs. For regulated industries, untracked AI is ungoverned AI — and ungoverned AI is a regulatory liability. Inventory completeness requires IT, procurement, and business unit collaboration, not just a data science team survey.
Generic risk tiering criteria (complexity, scale of use) miss the regulatory dimensions that matter in regulated industries. A low-complexity rule-based model used in medical claims processing is high-risk because of its regulatory context, not its technical characteristics. Risk tiers must incorporate sector-specific regulatory exposure, not just technical risk factors.
Monitoring AI performance generates data. Monitoring with defined thresholds generates governance evidence. "We monitor model performance" is not a control. "We monitor model performance against defined accuracy and bias thresholds, with an automated alert procedure and a named responder when thresholds are breached" is a control. Examiners ask about the latter.
The most pervasive failure: organizations complete the framework mapping and stop. They produce a document that says their existing activities align with NIST AI RMF functions. They do not produce monitoring logs, incident records, model validation reports, or governance committee minutes. NIST AI RMF alignment is not demonstrated by a mapping document — it is demonstrated by the operational evidence that governance is functioning.
